Form must be completely filled out by the applicant.
Applicants for the Tulsa Police Reserve must put (Reserve) after their name in the form.
Answer each question completely and accurately. Each blank must have an answer. If the question does not apply to you or the answer is none, write N/A in the appropriate space.
You must provide a valid email address at the end of the page. If you do not provide an email address the online application will not be processed.
