This online reporting system is provided for user convenience to report certain crimes that do not require the presence of a police officer. Use of this online system is entirely voluntary.
To use this online report the crime must have occurred within the city limits of Tulsa. Reports CAN NOT be accepted for other agencies.
You may only use the Citizens Crime Report if you are reporting one of the following crimes:

Forged documents or checks involve physical evidence and may be completed at a Uniform Division.
If the crime does not meet all the criteria above, or if you have questions, call dispatch--911 for an emergency or 596-9222 for non-emergency and have an officer sent to your location.
The blue
on the form provides helpful information for a particular field in the report. Clicking on the
opens a box containing further information.
When your report is processed in Records Division a number will be assigned and emailed to you. You must provide a valid email address in order to receive a report number by email.
When you receive the number the report has been accepted and is on file.
Making a Supplemental Report Online:
Use the Supplemental Report to document additional property loss.
The supplemental report can only be used if you have previously completed a police report, either online or on paper, and have an Incident Number. The Incident Number is a four digit year and six numbers i.e. 2002123456. You cannot make up an incident number or leave it blank. The Tulsa Police Department cannot accept Supplemental Reports that do not have a matching Incident Report.
The Incident Number is in the lower right corner or in the margin of a printed police report.
